Privacy and Cancellation Policy

 I acknowledge Eleni Hayek BHSc Naturopath, trading as Nerthus Health, complies with the Privacy Amendment (Privacy Sector)  Act 2000, the Health Services Act 1998, and the Health Records Act 2001 with respect to the privacy  of the individual and personal information provided int he course of the patient intake and  Naturopathic consultation processes.  The information provided by you will only by shared with your consent, or in compliance with legal/  regulatory requirements. As noted above, information is collected to gather clinical information  necessary to provide naturopathic care, maintain communication and follow-up, confirm  appointment bookings via telephone, SMS, or email, or, with your specific consent, the sharing of  information with other healthcare providers to enhance your treatment outcomes.  Please note a 50% cancellation fee will be applied in the event an appointment is missed (no show),  or cancelled with less than 72 hour notice.  Please be certain to advise of any religious, ethical reason or condition (present and/or suspected),  which may impact upon prescribed treatment. Some nutritional supplements and herbal  formulations may contain alcohol, meat/dairy by products or allergenic substances such as gluten or  crustaceans.  Use of natural medicines may cause an unforeseen, idiosyncratic reaction or the temporary  aggravation of a condition. In the event of an adverse reaction, please CEASE of the product(s)  immediately and contact the Naturopath. In a medical emergency dial 000 OR attend the Emergency  Department of the closest hospital.  Whilst seeking to work co-operatively with your other healthcare practioners with the aim of  enhancing your health outcome, a naturopath IS NOT A PRIMARY CARE PRACTITIONER. Treatments ARE NOT intended to replace care and treatment provided by your primary care practitioner.  Client consent: It is understood that I may at any time withdraw consent for the use and disclosure  of my personal information (excepting fulfillment of legal obligations).  I acknowledge that I have read and understood the content above in its entirety.  

Cancellation:

• A 50% deposit is immediately required with an online booking.
• A 50% deposit is payable 72 hours prior to your appointment if booked via the clinic.
• If payment is not made 72 hours prior to appointment, the appointment may be cancelled.
• A refund of your deposit will be given if the cancellation is more than 72 hours prior to appointment.
• A 50% cancellation fee will be held, if the cancellation occurs within 72 hours of the appointment.
• One reschedule allowed per deposit with 24 hours notice. Extenuating circumstances will be considered on a case by case basis.
